Polk Vision Awareness Survey

Polk Vision, Inc. is seeking to measure community-wide awareness of its function and goals through a survey. Your input will be invaluable in helping us set organizational priorities moving forward.

The survey is confidential! However, at the end of the survey, you can enter your name and e-mail for a chance to win a 32″ flat screen V, passes to local attractions, and restaurant gift certificates.
